ABSTRACT

The aim of this study was to describe a method for the induction of experimental secondary biliary fibrosis (SBF). Forty-seven Wistar rats were submitted to hepatic duct obstruction (OB group) for thirty days without ligature, section or cannulization causing interruption of biliary flow. This technique was carried out by simple traction of the bile duct passing it through the xiphoid appendix. Nine rats were submitted to a sham operation for bile duct stricture and seven rats comprised the control group. Blood samples were collected for the measurement of total bilirubin (TB), alkaline phosphatase (AP), alanine aminotransferase (ALT) and aspartate aminotransferase (AST). Liver fragments were removed for morphological study. Thirty days after surgery TB, AP, ALT and AST levels were significantly increased in the hepatic duct ligation group compared to the sham operated group and the presence of SBF in the OB group was confirmed by morphological study of the liver. There was technical failure in 31.92 percent cases. The survival was 100 percent at fifteen days and 82.97 percent at the end of the experiment. We concluded that this simple surgical technique may be used to study the consequence of bile duct obstruction which could be a reversible process depending on the obstruction time. This technique can be carried out from cholestasis to fibrosis.

ABSTRACT

Traumatic injuries of the extrahepatic biliary tract are infrequent, occurring in approximately 0.5 of all patients with blunt and penetrating abdominal trauma. The incidence of this injury due to blunt abdominal trauma is rare. This study reviewed patients with injuries of the extrahepatic biliary tract due to abdominal trauma over a 6-year period to determine the incidence, trauma scores, associated injuries, surgical treatment performed, complications and mortality rate. We report our experience with 14 patients with extrahepatic biliary tract trauma. A review of the literature and the discussion about the management are presented.

ABSTRACT

A lesäo traumática da vesícula e trato biliar extra-hepático é pouco freqüente e de difícil diagnóstico pré-operatório. OBJETIVO. Analisar, retrospectivamente, os pacientes com trauma abdominal operados em caráter de urgência no Pronto Socorro do Hospital das Clínicas da Faculdade de Medicina da Universidade de Säo Paulo, no período entre 1986 e 1991. MÉTODOS. Foram identificados 45 pacientes com lesäo da vesícula e trato extra-hepático e divididos em dois grupos: 12 pacientes com trauma abdominal fechado e 33 com trauma abdominal penetrante. Foram analisados, comparativamente, nos dois grupos: sexo, idade, índices de trauma, tratamento realizado, evoluçäo pós-operatória, lesöes associadas, incidência e mortalidade. RESULTADOS. A mortalidade global foi 24,4 por cento. Dentre as lesöes associadas, as hepáticas foram mais comuns (89 por cento dos pacientes). A incidência total foi 0,89 por cento dos pacientes com trauma abdominal (45/5.069). A incidência foi maior (1,25 por cento) nos pacientes com trauma abdominal penetrante (ee/2.650, em comparaçäo com 0,5 por cento decorrentes de trauma näo penetrante (12/2.419). CONCLUSAO. Os pacientes com trauma abdominal fechado apresentaram índicies de trauma estatisticamente diferentes daqueles com trauma penetrante e indicaram maior gravidade deste tipo de trauma. Isso fornece idéia de que existe correlaçäo entre intensidade do trauma e ocorrência de lesäo do trato biliar no trauma abdominal fechado. No trauma penetrante, a lesäo ocorre devido ao trajeto do elemento vulnerante e näo esté relacionado à intensidade do trauma. A mortalidade estatisticamente maior do primeiro grupo confirma esta idéia

ABSTRACT

O autor apresenta a análise de 63 pacientes vítimas de lesão traumática das Vias Biliares Extra-Hepáticas, ocorridos no Hospital Municipal Souza Aguiar (H.M.S.A.) e no Hospital Universitário Clementino Fraga Filho (H.U.C.F.F) no Rio de Janeiro. Não foram analisadas neste trabalho as lesões iatrogênicas. No H.M.S.A., em 85 (por cento) dos casos, a lesão foi da vesícula biliar, 13.3 (por cento) do canal colédoco e lesão do duto hepático comum em 1,7 (por cento) dos casos. Lesões múltiplas da via biliar não foram encontradas. No H.U.C.F.F., tivemos um caso de lesão múltipla da vesícula biliar, associada ao duto hepático direito e esquerdo. Os outros dois pacientes tinham o colédoco lesado. Dos 63 pacientes, 16 faleceram, na sua maioria, em consequência de complicações sépticas ou hipovolêmicas devido a lesões concomitantes de outros órgãos ou grandes vasos. A revisão da bibliografia nacional e internacional foi realizada, e a ausência de grandes séries publicadas justifica essa tese.


The author reports 63 cases of traumatic biliary extra-heptic ducts injuries, seen at the Hospital Municipal Souza Aguiar (H.M.S.A.) and Hospital Universiário Clementino Fraga Filho (H.U.C.F.F.) in Rio de Janeiro. Iatrogenic lesions were not analysed in this report. At the H.M.S.A., in 85% of the cases the gallbladder was involved, 13.3% common bile duct and 1.7% the hepatic Multiples biliary duct injuries were not found. At the H.U.C.F.F., one case of multiple injuries was reported (gallbladder plus right and left hepatic duct). In two other cases only the common bile duct was involved. Sixteen of the sixty-three patients died. Septic and hypovolemic complications, consequence of associated visceral or vessels injuries, were the great responsible of this hight mortality. Brazilian and International review was carried out. The small number of big series published about this subject justified this thesis.
